Without chatlogs it's hard to tell what the problem was. But fresh accounts are typically judged a little bit more harshly just for situations just like this one, where permabanned players that have learned nothing from their previous permaban just make a new account and exhibit the same behaviour that got them permabanned before. Just because you make a new account doesn't mean you get four new chances to exploit.

When you connect to Riot's servers they are able to identify your IP address which is basically your home address relative to your Internet Service Provider.

If you have had previous accounts that have been banned that accessed via that IP address, and you exhibit similar behavior, they are likely to escalate the process because it is very likely that you are the same person who is trying to work around their ban.
