I'm sorry to bring bad news, but permanent bans are not lifted unless there is some error on Riot's end. You had multiple warnings in two chat restrictions and a 14 day ban (unless you showed zero-tolerance behavior that caused you to skip the chat restrictions), and the reform card for your 14 day ban specifically said that your next punishment would be a permanent ban. You should have listened to those warnings and stopped your behavior before the permanent ban.
