Every game where a player is reported at all (whether by one person or nine people) gets sent to the behavioral system for review. The number of reports doesn't matter at all: all the system knows is that it was sent a game, and it should look at the behavior in that game.

If there is no problematic behavior, there is no penalty and no strikes against the reported person. If there IS problematic behavior their internal toxicity marker rises, and, if it surpasses the point of punishment, a punishment is applied.

If you feel you have been punished in error, I would encourage you to post your chat logs so we can help you see what you might have missed, or whether there may have been an error (which, while extremely rare, do sometimes pop up).

Yes, they scan the chat of those games and hand out their punishments accordingly.

No, premades don't effect you getting punished. It only does one review of the chat log, regardless of how many times people hit the report button. A single solo player's report matters as much as a whole premade. We've seen several examples on the boards were a 4 man premade gets punished because they were reported by the one solo.
