Just curious, how many games do you have to leave champion select/leave the actual game in order to get banned? How many times do you have to play games without leaving/dodging to get rid of your stacks and warnings?

Look up the "Leaverbuster" page in the news archives, should have the specifics.
I don't think leaving gets you banned. From my experience you just get 5 minute wait times to 20 minutes. 20 minute waits being the max. The only way to get banned I think is just to be super toxic, feed on purpose, and hack.
You cannot get banned for dodging.
Barring a medical emergency, you should never leave a game, period. The leaverbuster is sufficiently forgiving that you should never have to worry about it, provided you are not intentionally leaving games.
Thanks guys! Sometimes I crash in both Champ Select and Game Start and it concerns me whenever this happens. I know this is no excuse according to the Leaverbuster site due to it being preventable, but according to some posts Leaverbuster levels go away after a while if you're not dcing from any games in a period of time. If this is true pls tell me since that would be helpful and thanks again and sorry for the paragraph.
