If the punishment was deemed to be issued in error, they may overturn the punishment. These instances typically include a double dip in the system (triggering a 14 day ban but getting into a new game before ban drop AND being toxic in the new match), over zealous cases with newer accounts, or when a new strategy comes out that triggers a gameplay offense (like Inting Sion did).
Other cases include Reform (unperma) Experiments Riot may choose to conduct. (2 known cases thus far to my knowledge.)
it can happen but it's on the same probability magnitude as winning a triathlon the first time participating and without any previous preparations. riot isn't known for showing mercy to flamers . but it can happen.
Mostly bans were lifted for those players who paid for smth or got gifted ietms (payment issues).

Yes, one person from an experiment ,the experiment gave the permanbanend players a second chance, only one person made it as far as I know. Sadly the experiment was a fail :p

I’ve seen a few.
Most recently, players got their accounts back after writing in hand written apologies during the L10 event.

It depends, I've got a friend that got condemned for using cheats. He never used any addon for league of legends, neither used cheats, he doesn't even play ranked. Riot refused to listen and closed a 6 years old account.
I guess it's easier to ban nice players having been there and supporting you for years to make them buy everything again ? Instead of banning true trolls, people ruining games on purpose.
