The reform card isn't indicative of the actual behavior. You were intentionally feeding as Nunu by running it down bot and then AFKed.
You have a history of this type of behavior as well as playing Kench, and throwing people into the enemy. I will be escalating this to a ban.

The system does pick this stuff up. However, it isn't always easy for the system to decide what evidence to give in cases like this.

OMG! It never fails to surprise me!! This is somewhat depressing to witness, it's almost like it hurts to how cringy it is. Look on the audacity on this one!! The amount of nerve!! He knew what he did, he comes here to the boards fully knowing what he's been doing and that it could very well be known by Riot or easily found out the moment he came here..... yet he makes himself look good up until the Rioter shows his true colors!! Hey man, post a ticket. I recently got a crazy BS 14 day ban, sent it in and Rito replied swiftly and politely and revoked it :)
It could be that you were reported for toxicity in other games and this was the final game that you were reported for so they showed these logs.

That's a new one. You'll have to let us know what they say as a result of the ticket. I'm really curious as to whether this was just a glitch, or if there was something going on in the lobby. Hopefully it's just a glitch and they can get it removed for you. Best of luck.
