So if im playing in a game with only friends and no one reports me its still held against me later?

Title, I was in a game with only friends so I was vulgar and so forth no one reported me we had a nice fun game but it was counted against me when I got a chat restriction later because I said report karthus one too many times after he flamed me non stop for 40 minutes

If the game was referenced in your chat logs given to you in your reform card - someone reported you in that game. That is our understanding at least - the reform card will not show games you were not reported in.

LMFAO XD that really sucks but the enemy's might have reported you if they didn't see that the Karthus was being toxic to you. So they would think that you're being toxic blaming your team for loosing or something. Basically the enemy's probably only knew half the story.

I've always been under the impression that if you didn't get reported, Riot does not acknowledge it. But maybe they do... idk. Doubtful, but I hope a rioter let's us know. It doesnt seem like a very confidential part of the process.

Tantram has said that the IFS only checks games that are reported and it's also boolean, ie is the game reported or not (regardless of how many reports, ie 1 report in a game gets the IFS to review and so do 9 reports in a game)

Based on Tantram's statements, it is my understanding that if you weren't reported, the game doesn't get reviewed and doesn't count against you in the future. If you were reported, depending on what you said, it may be held against you.
