EDIT: Please criticize constructively. Not the "yes. deserved." comments. How should I type/what should I say instead? Yes, some salt, but I don't think it's ban-worthy.

Lots of people get banned because of their chat and post their logs on the board. The more you talk, the less you play. Your teammates are not your enemies, you win together or lose together. Advice is to refrain from chat and focus on your own gameplay. Don't put down your teammates. Don't post their KDA, CS score, how far they are behind. Unless you premade and duo, odds are low that you will ever play with that player again, and they can just be having a bad game. or a bad day. Re-read your chat log and imagine your teammates doing that to you. No one wants to be on the receiving end of that.

This is why chat should be fucking disabled in-game. He didn't say anything worthy of permanently losing the work he put into leveling his account. The protocol for handling stuff like this is insane. Should people who get more than three speeding tickets be forced to stop driving permanently?

It doesn't even stop the truly toxic people from being toxic. They just make new accounts and act horribly to new players.

Before I get into it, what was your punishment if you don't mind me asking? The only reason I ask is that larger bans are usually indicative of past or consistent inappropriate behavior. Also, were these the logs provided when you were notified? Riot employees have stated before that even though you were provided a specific game does not neccesarily mean that was the only content which was reported/inappropriate.

Getting into your logs the first and most objective thing I can say is that asking for a report can be an actionable offense. Secondly, as far as my opinion goes, you seem pretty passive aggressive throughout the entire chat log. Directly from one of the Support pages:

Negative Attitude - This category is reserved for unhelpful or self-centered attitudes that go against the principles of a team-based game.

Saying things like this is so over, "you just int", and "U deserve to lose" fits into that category pretty neatly. My recommendation to you, and everyone, is to /muteall every game and maybe go so far as to turn off all chat in your settings. Pings are more than sufficient to communicate what you need/want.

At the end of the day if you feel like you were punished unjustly, then you should submit a ticket directly to Player Support.

It's an escalating punishment system. Each punishment after the next is worse. After a two week suspension, any negative behavior can earn a permaban. If it's something that would barely give you a chat ban on the first offense... then yes, it's ban worthy if you've had a two week suspension.

Learn to remove salt from your chat diet.

You're certainly insulting your jungler, blaming him for how the game is going:

As well as needlessly insulting the enemy TF. Chat's designed for working together as a team, and I honestly am not seeing how you use it to that effect outside of the first 2 lines. Yes, this chat is punishable, and if it followed a 2-week ban, then the permanent ban was the next step.

It's tough deciding when to gank or not to gank vs farming up. You and Jax disagreed on how best to win the game, and that's fine, but blaming him for his play is not necessary nor helpful to winning the game.

Never say things you wouldn't say if you were to be on a sports team or to your coworkers. You can tell your jungler that they should have ganked more but that won't really help you in the future because having the same teammates, especially in ranked (unless you're really high elo), is unlikely. Same logic to giving them advice. Most likely it will start a flame war and you might end up reported. If you notice that a teammate is being toxic early on in the game just don't reply to them and mute them.
