When you go to the main euw.leagueoflegends.com site, look at the top and you'll see "support" in the bar. Click that link and it'll get you to the support site. You'll have to submit a ticket and then they'll ask you verification questions (like personal account trivia questions) to make sure you are the original creator and true owner of the account.
