What are the punishments sequentially for being reported for toxicity? (EX: first punishment is _______, second is _____, etc)

For general verbal toxicity - y'know, flaming, negative attitude, etc. - the punishments are as follows (assuming a clean slate):

For egregious verbal toxicity (usage of hate speech or encouraging self-harm - again, assuming a clean slate):

For extremely egregious verbal toxicity (death threats, not sure what else qualifies):

And then for Gameplay toxicity (trolling, intentionally feeding):
