If you wonder why people weren't listening to you much it's because of your tendency to throw insults around when people don't do what you want. It's okay to try to be a shot-caller, but good shot-callers don't throw out insults and harass their teammates when things go south.

Verdict: Punish.

P.S. The fact that you're using numbers to go around the language filter just prove that you want people to see your insults/name-calling.

Conclusion: When things get tough you give up, by that I mean you spend more time tossing out hate speech and death threats and insults than you do actually playing the game. This is grounds for an escalation to a permaban status, and it should come as no surprise to you that your teammates don't want to listen to you when you've shown that you'd rather resort to name-calling and the like when things don't go your way.

Advice:

It's okay to try to be a shot-caller, but good shot-callers don't throw out insults and harass their teammates when things go south. Never wish death, cancer, etc. on others. Not only is it not helpful but it just paints you as a jerk. Calling for reports is considered harassment. 1) Because it only takes one report to flag a game for review and 2) because you're using the threat of a report to try to get your way.

My overall/final verdict on this case: Punish.

Well, the advice only matters if he can suck it up and think logically: This is how I win games, by coddling my teammates even if I feel like insulting them. Calling for a change of personality might be too late to get moving, he just needs to logically suppress his personality for the sake of winning.
