Random Ban?

I'm not sure what just happened. I was just playing a ranked match, won and then instead of going to the Summary Screen it disconnected me. When I tried logging back in I was given a message saying I have been permanently banned from the game. I have no clue why. I haven't been reported, I haven't done anything to my knowledge. I didn't even get a reason. The bad part is it says to check my e-mail and I don't remember which e-mail I used to set up this account. Can someone please tell me if they have had a similar issue?

If you received a Reform Card then check your game client for it (you'll see it as soon as you log into the game). Otherwise file a Support Ticket about your issue.

It doesn't let me log in or give me a reason.

I found the correct e-mail address connected to my account and there is nothing in there from Riot telling me why I am banned.

i didn't get an email either but i swore and got perma banned o.o

You'll probably see it on tuesday, based on the fact that it is the weekend and how busy they are.

It's the big Support tab at the top of the screen. Go there and you can file a Support Ticket by clicking on the Submit a Request tab at the top of that screen.
