How is this on reasonable grounds a perma ban. Like Riot? I get I was toxic but a perma ban??

You're looking at it wrong. Think of it like Martian-rules baseball. You got your fourth strike, so you're out. Riot does it this way because statistics show people who don't respond to the first couple of account actions aren't likely to ever change.

It's not that this log is the worst we've ever seen - it's not. It's that you've almost certainly had ample opportunity to stop being rude to people.

Looks more like a heated argument between hashinshin and any other player. No seriously. Hashtilt got in a huge argument with another rioter. Also after a 14 suspension,next thing is a premaban. I don't see hate speach though. It's probably because it's a machine not human. Not saying it's deserved or not.
