Permanently Suspended? May i know why?

Just found out my account was permanently suspended in the email it doesnt specify WHY i was perma suspended. I would really like to know why Riot could ya give me a reason? I have never had a suspension or ban before. i did how ever have 2 chat restrictions like last year i believe it was. But im not seeing why i am being perma suspended. Could you explain this to me Rito? Thank you.

If you log into the game client you should see a Reform Card showcasing your chat logs. Please post them here in their unedited entirety.
