You receive a perma-ban for behavioral related reasons one of two ways.

Consistently poor behavior, which you would have received plenty of warnings to stop before reaching the perma-ban step.

Extremely poor behavior, likely a variety of which you should already know that it was an unacceptable way to behave.

Nice Meme, but you can be escalated multiple tiers. You can even be escalated to tier 4 right away. Some players don't want a 5th chance but a third, or a second.
