Mismatched champion icons and names

I've noticed that every time I log in, there's always a wrong icon on a champion in the champion list. Same thing happens in the lobby. In either case, if I click on the champion, the description is still correct. If I close and reopen league, usually the incorrect one returns to normal but then another champion has the wrong icon. If I join a new lobby, another champion has the wrong icon and the previously wrong one returns to normal. You can see multiple incorrect ones in each photo.
