Anivia Passive locked out

I was on my second account (level 12) playing in aram as anivia when i died and my passive did not trigger. no cd was activated, and no egg showed up. throughout the next 7 minute i died 3 more times without my passive triggering. I have no idea what caused it but im thinking it has to do with the fact i was at 2 hp and received a shield from morgana to save my life. i got out of there then died a minute later and that is when te issue first occured. so what i "THINK" happened is tha the game thogt i died and triggered my passive but i wasnt dead so it got stuck on cd without showing a cd timer.
