Game crashing at load/game failing to load after champ select

I'm still getting this issue after the supposed bugfixes to 9.14.

Since patch 9.14 any game I attempt to play in 5v5 summoner's rift crashes at the load screen or immediately in-game after load-in. I thought it was with specific champs but it seems to just be completely random and frequent. My game is still crashing after running Hextech Repair Tool.

After champ select the load screen no longer pops up and the client immediately goes to a prompt with the reconnect button. Pressing the reconnect button does nothing but apparently generates a new instance of the .exe in my task manager.

all of the LeagueofLegends processes coincide with each time i tried to reconnect to the game.
