Recall Bug in 6.15 patch and Penta not counted

As you can see in the link a Penta should have counted because there wasn't much duelling in between. Secondly and more worryingly, the game is seemingly showing a bug when you are entering a fog of war at the moment a champ starts recalling. Him and sometimes champs around him disappear from map.
