(Gameplay) Cannot use TP summoner spell

Teleport Summoner spell was blurred out after a death, It was not on CD, just said this spell cannot be cast right now. It appeared as though the game thought i was still under CC, had died after a shen taunt and pantheon stun. Happened during a Normal game playing as Aatrox. Have not tried to reproduce.
