Screen Black out after Champion Selection

I teamed up with my friend, everything was good until we finished champion selection. The screen blacked and shows this pic as below. I can never reconnect until this match ended.

I get the same Exact problem It always happens to me i can never play with my friends and stuff ive read so many guides and tried them nothing would work like nothing So please can i have some feedback maybe on what to do

Please riot fix this issue for the next Patch

i got the same error in the middle of a game during a fight tried to reconnect several times but kept getting the same error
