Okay so my problem is solved, and I don't know if your problem is the same, but here's everything I did and I haven't had a single crash in over a dozen games at this point.
Attempt 1) The usual fixes - right click LoL icon and clicked "troubleshoot compatibility", repaired, and removed (and rebooted) then reinstalled (and rebooted) LoL. No fix.
Attempt 2) My Catalyst Control Center wasn't working right either, so I uninstalled (and rebooted) and reinstalled (and rebooted) CCC. No fix.
Attempt 3) Uninstalled (and rebooted) then reinstalled (and rebooted) .NET framework. No Fix. As a side note, I had to repeat six windows updates which were apparently to .NET... go figure.
Attempt 4) Completely uninstalled (and rebooted) all AMD drivers, then installed (and rebooted) the chipset driver first, followed by installing (and rebooting) the Catalyst Control Suite. Fixed.
On another side note, they (AMD) released the new "omega" drivers on December 8th or 9th for both chipset and gpu, and it's possible that the fix was in this update. I'm running a Gigabyte 990FXA-UD7 rev 3 mobo with dual Sapphire Tri-X R9-290X GPUs and had been having issues with crossfire previously - those issues are gone as well now.

I get the same thing. It happened around 4.15 in PBE (when the new SR map hit the FoJ) and I stopped playing the new map on PBE for a while for it. That's not really an acceptable option in the live environment though, and I have no idea what they're doing about it. I have done repairs, complete reinstalls, and the "right click the icon, click 'troubleshoot compatibility' fix" and nothing has worked for me. At least there's a bright side, you can play ARAM instead of ranked. I haven't seen any crashes that I can recall on other maps, I might have had one or two in ARAM but I don't believe so. I also hear that custom games won't do it on SR, but TB games will. I haven't tried TT or Dominion, though I imagine they work too because the problem seems to lie within the new SR map release.
Im getting the same problem :3 only happens on summoners rift
I went back and verified, PBE SR doesn't crash for me. Whatever the fix is for this error, it will likely come soon.
