Couldn't pick champion in ranked champion select [CRASHED]+ My item disappeared [GAMEPLAY]

First i was playing Taric and bought the green gold item and started farming for my carry. Then i back to base to get the next level of the item and went to farm again with 3 charges. I was with low health so back again and when open the shop i saw the Targon's Brace was not in my inventar so i had to bought Relic Shield again and start building it again. The bar icon for the charges of the item wasn't there and appear when i executed 2 minions. The second problem was while making my next ranked game with my cousin (account: ofanzivniq). A window with message "Retrieving data from server" appeared and nothing happened. I couldn't pick my champ and when the time over i was turned back to the main page and i had a penalty for quit the game so i had to wait for another game. Before the time was over i checked a few times my internet browser and i had connection to the internet but in league of legends i had black screen and a small clock as a mouse pointer.
