Crafting: Chests
It says I have all four chests available, but everytime I get an S or someone gets an S I do not receive a chest. I keep getting key fragments though, so I do not know what is going on. I also do not have a ban either. Halp.

I have the opposite problem, too many chests but not enough key fragments :(
You have to own the champion and you couldn't have gotten a chest on that champion before. Go on your profile and click the champions tab. Each champion you have received a chest on should have a golden border and when you hover them it will have a chest icon on the bottom right of their info tab.
