Duplicate draft?

I wasn't able to get a screenshot unfortunately - because I restarted my client before I had the thought, but...

I duo-queued with a friend of mine and we got into a game, nbd everything was fine - but I locked in my Rengar jungle and swapped windows over to change up some music and when we got in game I had no health/ferocity bar, couldn't see summoner spells and when I hit tab I could only see their Cho'gath and our Ahri, but the cs scores were jumbled up so it was just stacking the players in the menu.

With 100% certainty, I got jungle - when we got in game it was brought to my attention that I got top and the Vi had gotten jungle even though we were in draft pick. Anyone else experience this issue? Is it on my end? (We lost, by the way.)
