Tahm + Quinn perma ulti bug

Im not exactly sure what caused this but ive just played a game as Tahm were at some point I used devour on my adc Quinn who was in ulti form. And some how (I think he died inside) we both died in the team fight and when he respawned he was stuck in bird form and was stuck like that for the rest of the game. Was really weird.
