chest loot - failure to add to loot collection

1. After finishing a game and receiving a key frag, I forged a new key.
2. Used said key on my single hextech box.
3. Loot reward showed as Heartseeker Ward.
4. Clicked add to loot (where I already had 2 Heartseeker Wards).
5. Remaining total still at 2, Essence remains unchanged(ie, no auto disenchant).
6. Re-logged: still no change in loot lab.

(current patch 6.17.16_08_18_12_24)

bug is either failure to add to loot or too many dang ward skins :)
