Inventory Sight Stone ERROR

I was playing Taric, and my sight stone stopped getting charges. It happened after I died. (Picture is of me at fountain with no charges on my Sight Stone)

This happened to me last night in a game, I was playing either Morgana or Karma. Can't remember which game exactly without pulling up the replay.
