This bug has been known for a while, but basically, when Ahri's Q returns, it doesn't always hit the target. I've noticed that it happens with minions the most. (However, it does happen to champions frequently as well.) i.e., farming in lane and you Q a wave, 4 or 5 minions out of 6 get hit by the returning Q. I'm not exactly sure if there's a way to reproduce it 100% of the time, as far as I know, it's random. It happens about 3/10 times.
