[Gameplay] Unable to do anything upon death

This started happening about two patches ago, seems to happen upon death but I don't know how to directly replicate it.

In a game everything will be working perfectly fine with no issues then I die at some point.

Once I spawn I can't do anything but press enter, type, and press enter again. Escape key doesn't work, can't click anywhere or on anything. I can cast SOME skillshot skills but not all of them, but I'm using quick cast and it does not quick cast, it gives the standard indicator and then can sometimes cast but most of the time does not.

It is permanent unless I quit through Activity Monitor and try to rejoin the game.
