Holes opening in map [GAMEPLAY]

I have been getting holes in the map constantly opening and closing at a rate in which could endues seizures to people prone to them. This has been happening since around the start of season five and I have finally gotten an image to go with the report.

These holes appear at random every couple of games if I where to put a number to it I would say If I found a hole in the map within the next 2-5 games I will see another set of holes opening

These openings only occur once a game so in the screen shot above the holes that opened where the only ones that game

I cam unsure what causes this bug and thus cannot tell you how to reproduce it but it would still be appreciated if some consideration was put into fixing this
