Happened to me last season. You probably was toxic and they took your rewards. You'v probably been banned recently after August 9th of 2015

nope i not only lost my season 4 rewards but they took everything all the way back to season 1.

I reformed during mid season 5 and didn't get banned anymore. So they did give me my season 5 rewards. It was during season 4 i had a bad attitude. I still kind of have a bad attitude but i try to ignore people or just stop playing when i go on tilt. The game in its state is super frustrating.
