[CRASH] Full game crash (once you enter S.Rift) when playing as Yasuo

Details: Summoner's Rift 5v5, blind pick. When I select Yasuo (specific, I know), the game moves to the loading screen successfully and loads to 100%. Once the actual match begins, the game completely crashes and it requires a full computer restart. In the past 4 Yasuo games I have played, this exact crash has occurred.

It seems too strange, but when I select specifically Yasuo it crashes everytime!

Sequence: (1) Select Yasuo, rune pages, skins, etc. (2) Countdown finishes, loading screen begins. (3) Once loaded, enter Summoner's Rift - I can hear the music, and see the nexus circle on a frozen screen, but no champions appear. The game hangs frozen here indefinitely. (4) Cannot exit game in any manner; must Alt-Tab to Windows (no other programs can be maximized either) and restart the computer. (5) Upon restarting and logging back into LOL, can "Reconnect" to the match and play normally.
