The title says it all but the heal procs if if i last hit the minion, but it doesnt proc the execute passive. I have to wait til the minion is extremely low. (Yes I know that relic shield has regenerating stacks, I had fuil stacks when this happened). It happened in multiple games.

thresh is an range auto attacker the execute on relic shield applies for melee attacks therefore you have to LAST HIT the minion as thresh and it will proc the gold passive

Try reading the first sentence in Relic Shield's tooltip.
Hint: Thresh is ranged.
Intentional and not new. Thresh is considered ranged.
Relic Shield is still the preferred starting item on Thresh because he has THE LOWEST ARMOR of any champion in the game, and he uniquely does not gain armor by level. Having early health is a huge benefit to him early game. He could get a different health item, but the fact that this is a gold income item makes it the ideal start for him.
this has been an intentional part of his kit since his release...
also this why some people take attack damage on thresh as a support to make his last hitting easier for relic shield and to help his harass as well also try to proc the relic shield with the passive from E giving you more damage to make it easier to last hit
