Annie Q and R not responding
Annie's Q didn't respond in the middle of the game and R frequently didn't respond as well. Keyboard is fine, it didn't happen when I play other characters.

Happened to me as well in a few games. Also noticed her stun doesn't always go off with Tibbers, directly on top of 2 or more enemies. None of them built anything at that point which could stop it. It happened quite a few times in Poro King.
R not working has been happening to me on a few champs, namely malphite and evelynn.
