Aatrox passive bug

I haven't player Aatox for a while. I knew when you get near death experience his passive should prox and you had a chance to go back in the fight. However, I played a game and out of 13 deaths, his passive activated only 1 time. I went to re-read his passive for changes. It said "when on low health". what does that mean 1,20,50? I had the expeience that even on low health his passive didn't activate at all. One time out of 13 looks like a solid bug to me.

The passive only works if your bloodwell is full (the red bar under your healthbar). You get stacks for using q,r, procing the damage version of your w or hitting enemys with ult activation.
