Removed from game?

I was playing a game as jungle and it disconnected me. When I exited the game and tried to reload/relogin, it took me back to the League of Legends home page. I can't rejoin the game, it doesn't show up in my match history, and it let me into a brand new game. It's like the original game never existed.
