Quinn (stuck as Valor)

Whilst in a team fight, I ulted into my Valor form. When I died, I died as Valor, unable to revert from my ult to do damage. I may have died as Valor in Tahm Kench's stomach, but I don't recall (their team had a lot of CC so it was hard to really pay attention how you died.) After I respawned, I was still Valor, I could not revert and my ult was stuck at a 97 second cool down. This did not change during the entirety of the last half of the game, causing us to lose. (I'm not saying it's the reason we lost, but I was doing very well before I got stuck in the melee-bird form.) Not sure if this will get looked into as there seems to be a Quinn rework coming up. But just thought I would bring this to Riot's attention.

The team I was playing against was Azir, Elise, Leona, Vayne, and Volibear. I had Tahm Kench on my team as stated previously. So I'm not exactly sure what could have caused me to get stuck as Valor, but hopefully this information helps.
