Tahm Kench w bug

I just finished a game as Mordekaiser in top lane against a Tahm Kench. He was able to spit me out like a minion into his tower when he got 3 stacks applied on me. Please do fix this, it only happened once this game and both Tahm Kench and I didn't know how it happened but please take a look in it I would greatly appreciate it.
