Darius Q not applying damage and heal on the blade

Played a game as Darius against a pantheon top and periodically Darius q didnt apply any damage or a heal, and cost me a few trades. idk if it's bugged or the pantheon hitbox is broken but idk. i dont know what the conditions were or if it is recreatable. i saved the replay, i just cant figure out how to upload it

Upload the replay to YouTube and post link. I had the same problem in my darius games and when I zoomed in and watched in slow motion, it showed the axe part didn't actually hit but the after image did, which is why it looks like it did.

I don't think it's a bug as much as a clarify issue on his q animation
