I took a look at your match history and noticed that the game was an ARURF game. You can only earn tokens on normal or ranked Summoner's Rift matches. You cannot earn them on ARAM, Twisted Treeline, or any of the rotating game modes.

Same happened to me when I got an S on Kindred but I wasn't lvl 6 yet so I didn't got the token too. That sucks.
