[CRASH/GAMEPLAY] Multiple DC's; summoner match histories not matching up (Riot please read)

In a recent normal draft game I kept getting random disconnects. The game would show "Attempting to Reconnect" sporadically throughout play, even though ping was consistently at 75-100, and there was no indication of lag or other performance-affecting issues prior to and after the error messages show up. I was playing as Vi and another thing I noticed was the inability to use my Q in the middle of the game (whereas just seconds ago it was usable). I was playing against a Riven and was pretty sure that she didn't stun me while I was channeling my Q prior to it happening. Attached image shows my ping at 92 while the error was up once.

Now I'm never the kind of person to point fingers and claim "LOL DAT PERSON HACKED FML REPORT NOW", but with the sudden disconnects happening after finishing a smoothly running ranked game earlier today, I did get suspicious.

So I pulled up the Riven's match history: http://matchhistory.na.leagueoflegends.com/en/#match-history/NA1/205264801

which, while being hilariously improbable, also does not coincide with what is shown in the client's match history (which looks like this): http://na.op.gg/summoner/userName=NetChaos

Also, if you click on one of the Udyr games in the former Match History page, the name pulled up will be Eightbit Srk instead of NetChaos.

I just want Riot to look into stuff like this.
