Not earning mastery tokens
This problem have occurred twice with me, i was playing on the rotation game mode(Legend of the poro king and hexakill), got and S-/S and didn't receive the token. I didn't won the game but still i should've got the token.

You can only get the mastery tokens from normal and ranked games on SR. You cannot receive them from any other game mode. I believe this was a recent change made last week starting with Poro King.
