Quinn stuck in bird form in NA live summoners rift

Ulted as quinn then got eaten by tahm kench. After that I was stuck in bird form for the rest of the game. Dying/reloading the game did not solve this and tahm kench eating me again did nothing. Ult was greyed out and unuseable. Thanks.
