Sona's passive stack is still bugged :c

Occurs 100% of the time when casting a basic ability 0.5 seconds (or some weird time frame) after Passive Chord, not giving you a passive stack. This means that you need to use 4 abilities to use the same Passive Chord. Which ruins her ability rotating, Mana and time to use the next passive chord.

This bug has been around for over a year or more, for someone who spams and mains Sona, it's a bad habit that I have to play around it or be careful, since from the video it's easy for it to occur.
