A very serious champ select bug!!!

It happened to me 2 times already, when I try to start a blind pick match, it finds me a game and after I press accept this is what happens... http://prntscr.com/gqnbz9 and after the champ select timer is over I get stuck in even a wierder zone! http://prntscr.com/gqndcf
