Hextech Chest Bug

This may already be known, but I've seen this bug several times in my own games throughout the last several weeks. I thought I would go ahead and post it here for more visibility since it hasn't been addressed.

I've noticed that when I earn an S- or better that I do not receive a hextech chest if I complete a mission in the same game. This has happened to me several times over the course of the last several patches. I've submitted this information through the bug report system, but I wanted to post here for more visibility since the bug reports are being overlooked. I have a hextech chest available according to my profile. I've made sure to check so I know it's not that I don't have a chest available.

Are you sure that you might have just already gotten a chest on that champ?

Is there a restriction on which champs can earn chests, and if so where would I look for that? My profile has a 1 underneath the chest icon and says "Get an S- or higher by yourself or with your premade to earn chests". It doesn't say anything about selectivity with respect to champs.
