Champion Select bug

Not sure exactly what's up with this but occasionally whenever I enter matches with a normal queue(teambuilder works fine) instead of bringing me into Champion Select as it should when I accept the match it sends me back into the lobby. But I can still hear the noises of champions being banned/selected, and it shows me in the Champion Select to other players.

The picture is of how it shows me in the lobby with someone else as if we're yet to queue, but how they're actually in Champion Select without me. Oddly it shows them with a default Summoner Icon instead of the one they actually use.

I'd really appreciate help with this as because of it I've already gotten 15 minutes queue restrictions due to it occurring along with the firewall disconnect error.

(Their name is blocked out just as a random privacy thing of not wanting them bothered by people who see their name on the boards. xD)
