Game vanished
I just played a game that disconnected everybody after 5 minutes. When leaving the game, i was presented with the normal launcher and could start another game with my 4 premades.

Something similar here, got d/c'd and ended up at the home screen with no way to rejoin my game. Not in my match history though.
