Not making it into champion select, but can hear sound in the background. Forced to dodge every time

I'm getting a bug EXTREMELY frequently recently (1/2 of the time I click accept queue). I can hear the sounds of champion select, I can hear the bans go back and fourth, I can hear when it's my turn to pick. But I am not in the champion select screen and it almost always forces me to dodge.

It seems to be more likely to happen (although I am not able to reproduce this bug at will) when I change windows/"tab-out" of the screen immediately after accepting the queue.

Any one know any solutions? Am I alone in this?
